So far, all i've figured out is 1. Jacob is mentally ill. 2. The Doctor does weekly checkups on Jacob. 3. The Smiley(s?) try to influence Jacob to do horrible things, and Jacob does it becouse of the happy mood the Smiley(s) bring.
